USD 220 - HEALTH GUIDELINES and PROCEDURES

Prescription Medications:  The request to administer medication form will need to be signed by the parent/guardians and the health care provider before the medication can be given by the school nurse or designee.  Once the form is completed by the parent/guardian, it can be emailed/faxed to the Health Care provider's office for signature if needed.

OTC (Over-The-Counter): The request to administer medication form will need to be completed by the parent/guardian only.  Doctor's signature is not required for OTC medicines.

*All OTC (Over-The-Counter) and Prescription medications must be provided in the original container.  
*All medication will need to have the student's name on the container.
